Who we are

This website is run by Dr Jonathan Cousins Ltd (company number 7550786). Dr Jonathan Mark Cousins is registered with the Information Commissioner's Office as a data controller, registration number ZA498729.

What we collect

When you use the contact form, we collect the details you choose to send: your name, email address, phone number and your message.

Please don't include detailed medical information in the form. We will discuss your health with you directly.

How we use it

We use your details only to reply to your enquiry and arrange any consultation or anaesthetic cover you ask for. The lawful basis is our legitimate interest in responding to you, or taking steps at your request before providing care.

If you go on to become a patient, your medical information is handled under the confidentiality and record-keeping rules that apply to all doctors.

Who sees it

Form enquiries are sent to Dr Cousins and are read only by him.

Our website host (names.co.uk / Website Builder) stores form submissions on our behalf. If you become a patient, our billing service, MedServ, receives the details it needs to invoice you or your insurer.

We never sell your information or use it for marketing.

How long we keep it

We keep enquiries only as long as needed to deal with them, no more than one month unless they become part of a medical record.
